Billing & Insurance

In Washington State, you do not need a physician’s referral to be seen by a physical therapist. However, with most insurance companies, for your visit to be covered, you will need a referral from your physician. If you don’t have insurance we will be happy to see you on a fee-for-service basis. Most of our patients have some form of health insurance. As insurance regulations are constantly changing, we ask you to be an active participant in determining your PT insurance benefits.

We are happy to bill your insurance company if we have your insurance information. To minimize confusion and surprises, we have developed an Insurance Benefits Questionnaire for you to use while talking with your insurance company. The answers on this form will provide you and us with important information about your benefits.

Payments can be made at our front desk, or mailed to us. We do not currently accept credit cards.

We are preferred providers for most insurance companies. Following is a list of some of these – please check with your insurance company for further details.

At this time, we have been unable to contract with United Healthcare.
